Self-Managed Abortion with Misoprostol: Trusting Your Body, Trusting Yourself

The World Health Organization’s 2025 Abortion Care Guidelines affirm what so many have known through lived experience: self-managed abortion with Misoprostol is safe, effective, and can be guided by one’s own body awareness.


This is what demedicalization means — not the absence of care, but the return of knowledge, power, and safety to the people who live in these bodies.

What the 2025 WHO Guidance says about Self-Managed Abortion with Misoprostol


For pregnancies up to 12 weeks, the Misoprostol-only regimen is:

  • 800 micrograms (four tablets of 200 mcg each)

  • Placed under the tongue, between cheek and gum, or in the vagina

  • Take every 3 hours, for three rounds — a total of 12 tablets over about 6 hours.


This process starts contractions and softens the cervix. Cramping and bleeding are signs the uterus is working. The body is doing what it knows how to do.


Listening to Your Body


You don’t need a doctor to tell you when the process is complete — your body gives you cues. You’ll likely feel or see:


  • Cramping and bleeding that intensify, then slow.

  • Tissue or clots passing — this is the pregnancy leaving the body.

  • A gradual return of normal energy and appetite as hormones shift.


When the pregnancy tissue has passed, the abortion is mostly complete. But sometimes, a little remains. You might notice:


  • Bleeding that stops, then suddenly becomes heavy again.

  • Persistent cramps that don’t ease.

  • A foul smell or ongoing pregnancy symptoms.


Your intuition will often tell you before any “clinical” sign does. You might feel that something isn’t fully finished — a heaviness, a lingering pressure, a sense that your womb hasn’t released everything yet.


When to Take Another Dose


WHO confirms what many have already practiced:

It is safe to take another 800 micrograms (4 tablets) if you believe the abortion is incomplete.


This extra round can help the uterus finish emptying. There’s no danger in trusting that instinct — it’s the same medicine, the same gentle push. Completion is both physical and energetic. You’ll know when your bleeding becomes lighter, your body feels calmer, and your center feels “cleared.”


When to Reach Out


Heavy bleeding (soaking more than two large pads per hour for several hours), high fever, or severe pain are uncommon but important signals.

If they appear, seek medical care — not because your body has failed, but because you deserve support.
